1. Self-Disclose
A student seeking accommodations for a disability must self-disclose their need for academic accommodations, auxillary aids, and/or services. This is done by completing the online Introductory Questionnaire. (Note: If you receive a proxy error or internal server error message when accessing the form, please use a private/incognito web browser to resolve the issue.)
2. Registration Paperwork
Once our office receives your Introductory Questionnaire from step 1, we will send you a registration packet to your PSU email address. Please read through the packet, complete it, and return it to [email protected]
3. Diagnostic Documentation
Each student is required to submit diagnostic documentation from the licensed professional currently treating you for the diagnosis you are seeking accommodations. Documentation Guidelines.

4. Intake Interview
Once the documentation from steps 1, 2 and 3 has been received and reviewed, you will be contacted to schedule an intake appointment with the Student Disability Resources Coordinator.
5. Accommodation Letter
If the documentation supports the request, accommodations will be determined. An accommodation letter will be sent soon after the intake interview. The student is then required to share the accommodation letter with each professor whose class you wish to use the accommodations in.
